H1N1 Influenza Information

H1N1 2nd Dose - Letter to Parents - December 8, 2009

Attention Flu Mist Recipients: “Screening Questionnaire for Intra Nasal Influenza Vaccination” prescreening form must be filled out in order to have the flu mist. There can only be one “yes” checked on this form – “Are you between the ages of 2 and 49?” If there is any other "yes" checked, the student will not be able to have the flu mist.
